The purpose of this survey is to find out if families are presently receiving services remotely via computer, tablet, smartphone, or telephone (known as "telehealth") and, if so, their level of satisfaction. The collected information will be summarized and used to make recommendations to the regional center and the State of California as to whether continued telehealth is recommended.
No personally-identifying information will be included in this survey and raters may decline to respond to any questions.
